I don't think there's been a definitive answer on what happens to the soul of someone who undergoes ceremorphosis, but it's pretty heavily implied to be unpleasant and worse than simply dying. In the upcoming Baldur's Gate III game there's a Cambion who's very happy to mock you that your soul is essentially worthless to him as long as you have a mindflayer tadpole in your head. Even if you kill the mindflayer that took over their body, and/or create a new one ( Clone, True Resurection etc) (unless that's changed in 4th or 5th, I'm using 3rd edition material) At all. to the Fugue Plane, at least not fundamentally unaltered.įor one, you can't return the dead person to life without a Miracle spell. After that point, Miracle is the only thing that will bring back the victim-I think this indicates that something more sinister has happened to the soul, as that spell is usually reserved for when a soul is destroyed or transformed or otherwise indisposed in such a way that the usual avenues of bringing someone back no longer work. I don't know if True Ressurection was a spell when the Illithiad was published, but Lords of Madness includes it as a solution, but only if it's used before the ability scores reach zero. However I think within the context of both books talking about which spells to raise the dead don't work after the process. So you could theoretically kill the mindflayer and True Resurrection would work, since the body is finally 'technically' dead, but it wouldn't as long as the mindflayer is still alive. I can see how that would interfere with bringing someone back from the dead, since their body is alive even if they are dead. Mechanically, the person is dead, but their body is still alive (albeit heavily mutated and puppeted by a parasite melded to their brain stem). Lords of Madness has an interesting take on that. I could see a special prayer or ritual or something to actually call back the previous soul in its old body and repair the transformation, but that's the DM's call for sure. Now if you kill the illithid and try to use its dead body to resurrect the original host, I think you would just end up reviving the illithid. IIRC prior editions allowed the use of Resurrection (7th level spell) without the body if you had a hair or blood, or even an object that belonged to the deceased, in which case that would also be an option. Its body however no longer exists, well in my opinion at least, just like if it was disintegrated so you would require a True Resurrection spell. What I'm saying is that if the process finishes, the host dies and can be resurrected, its soul is not trapped. That is to interrupt the ceremorphosis process. Until ceremorphosis was completed, the emerging mind flayer remained helpless. Internal organs were reshaped, but retained their original purpose. Over a period of approximately one week, the host's body underwent extensive morphological and tissue changes, completely transforming into a mind flayer.
